General Waiver, Release and Acknowledgement
The athlete and parent/guardian accepts and assumes full responsibility for any and all personal claims arising from this event and releases the Upper Arlington City School District, the program organizers, any officials and/or workers from liability or other catastrophe.
COVID-19 Waiver, Release, and Acknowledgment
In light of the COVID-19 pandemic, the athlete and parent/guardian accept and acknowledge that the athlete’s participation in the Upper Arlington Mat Cubs Youth Wrestling Program (the “Program”) is conditioned on the following safety precautions and acceptance of the following terms and conditions:
- Prior to participation, the athlete will conduct a daily symptom assessment (self-evaluation) and will stay at home if experiencing symptoms of fever (100 degrees or higher), cough, shortness of breath or difficulty breathing, chills, muscle pain, headaches, sore throat, or new loss of taste or smell. The athlete will stay at home if he/she has been exposed to any person who has tested positive for COVID-19 in the past 14 days. Any athlete with an enhanced risk of developing serious complications from COVID-19, such as those with asthma, diabetes, or other health problems, will advise the Program organizers of that enhanced risk in writing so that the Program organizers can adopt extra precautions to protect him/her.
- The parent/guardian and athlete understand that participation in the Program involves risks and dangers including, but not limited to accidents, illnesses, and death, including but not limited to COVID-19 and any related or derivative disease or condition. By participating, the undersigned hereby assume all risks of participation in the Program including those set forth above and release, discharge, and/or waive any and all liability, claims, damages, causes of action and/or demands against the Program, Program organizers, and the Upper Arlington City School District (the “School District”), of every kind and nature which may arise from or in connection with the Program. The undersigned further agree to indemnify and hold harmless the Program, Program organizers, and the School District from any claim arising out of or related to the athlete’s participation in the Program.
- The athlete will at all times of participation in the Program be covered by a current accident/medical policy.
- The athlete’s participation in the Program is conditioned upon the athlete and parent/guardian complying with all federal, state, and School District orders, rules, and regulations both current and as amended/updated, including but not limited to those related to health, safety, and/or the spread of contagious disease, including but not limited to COVID-19 and any related or derivative disease or condition and may be revoked at any time. The athlete and parent/guardian will comply with such rules and regulations at all times while the athlete is participating in the Program.
